The doors of our new state-of-the-art facility are open, and the Pennsylvania Center for Periodontology is welcoming new patients.
At your first consultation or comprehensive periodontal exam, our doctor and staff will take the time to discuss your concerns and help you outline your dental goals. Please fill out our new patient paperwork and bring it along to your first appointment at our office!
YOUR FIRST VISIT
Most of our patients are referred to us from their general dentist or doctor, but we’re also able to consult with new patients for certain procedures. When you call into our office, our staff will be able to determine if you need to schedule a consultation or are able to schedule your procedure date. This process varies with each person and your specific case.
If you are brought in for an initial consultation, our dental staff and doctors will go through a thorough analysis and determine your treatment plan, specifically designed for you and your dental health. Sometimes your procedure may be able to be performed the same day as your initial consultation, but you’ll be notified of this possibility before your appointment so you can arrange proper transportation from our office.
To help us make your first appointment the best experience possible, please provide the following information when you arrive (or beforehand if your referring doctor allows):
- Referral slip from your doctor
- Any x-rays from your general dentist
- List of medications you’re currently taking
- Your medical and dental insurance cards
- Your photo ID (for minors, the parent or legal guardian’s ID)
X-RAYS
If your dentist or doctor has taken x-rays, you can request that they forward them to our office. If the timing between appointments is too short, you can pick them up and bring the physical x-rays to our office for your consultation or scheduled procedure. If additional x-rays are necessary, we can take them at our office.
EXISTING MEDICAL CONDITIONS
Please alert our office if you have a medical condition that may be of concern during your procedure (i.e., high blood pressure, diabetes, pace makers, cardiovascular conditions, etc.) or you are currently taking medications.
PATIENTS UNDER THE AGE OF 18
All patients under the age of 18 MUST be accompanied by a parent or documented legal guardian during all appointments including consultations, surgical procedures, any follow-up appointments.
